As a member, you have Primary Care Provider (PCP) Choice.
When enrolled with Absolute Total Care, you and your family can choose the same PCP or each can choose a different PCP. It's completely up to you!

You should receive your ID card in the mail once you are enrolled with Absolute Total Care. If you did not get your card, or you need a replacement:
- Call Absolute Total Care Member Services at 1-866-433-6041 (TTY: 711) for assistance.
- Register and access our secure member portal. From there you can request, print a temporary ID or download a digital version of your ID card on your smart phone.
You can locate your member ID number on your member ID card. If you misplaced your ID card, please call Absolute Total Care Member Services at 1-866-433-6041 (TTY: 711) for your Member ID number and new ID card.
To update your address, call Healthy Connections at 888-549-0820 or online at apply.scdhhs.gov.

You can use Absolute Total Care’s Find a Provider tool to search through our directory for a primary care provider (PCP), facility, pharmacy and much more.

- Register and access your account through our secure member portal to change your PCP.

After you're approved for Medicaid, you'll receive an enrollment packet from Healthy Connections Choices with your Medicaid plan options. You will need to choose a health plan by calling 1-877-552-4642 or online at https://www.scchoices.com. Once you choose Absolute Total Care as your health plan, you will receive a welcome packet from us. Enjoy your new benefits!
