Wellcare Prime Medicare-Medicaid Plan Change Effective January 1, 2026
On January 1, 2026, the state of South Carolina is transitioning all MMP Healthy Connections Prime plans to exclusively aligned Dual-Eligible Special Needs Plans (D-SNPs). Therefore, the Wellcare Prime Medicare-Medicaid Plan (MMP) will change to a D-SNP named Wellcare Absolute Total Care Dual Align (HMO D-SNP). The new plan coordinates care for those with both Medicare and Medicaid under one parent company.
No action is needed by members already enrolled in Wellcare Prime Medicare-Medicaid Plan (MMP). Members will receive a new ID card in the mail. Members and Providers can visit the plan transition page for specific details on the transition.
